EXHIBIT"A" <br /> Scope of Services—FY 2014- 15 <br /> Outside Agency Performance Agreement <br /> Agency Name: Big Brothers Big Sisters Of The Triangle, Inc. <br /> Program Name: Community-Based And School-Based Mentoring <br /> Funding Award: $2,000 <br /> Outline how the agency will spend Orange County's funding award. <br /> Expense Description Amount <br /> This amounts to 7%of the salary of one Enrollment and Match Specialist serving Orange $2,000 <br /> County matches. <br /> Program Services <br /> For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the <br /> contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes <br /> below,by June 30,2015. <br /> • Carefully screened and trained adult volunteers will provide one-to-one mentoring to children <br /> enrolled in our Community-Based Mentoring program. Bigs will pick up their Littles at the <br /> Littles' homes weekly and participate in a variety of cultural, educational and recreational <br /> activities in the community. <br /> • Carefully screened and trained adult volunteers will provide one-to-one mentoring to children <br /> enrolled in our School-Based Mentoring program for the school year. Bigs will visit with their <br /> Littles weekly on the Littles' school campus. They will read together, do homework or play <br /> educational activities, among other activities.. <br /> • NOTE: The Anticipated Results of the Performance Measures below are based on serving 100 <br /> Orange County children. <br /> Anticipated Outcomes <br /> The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange <br /> County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants <br /> within that measure's description or for an earlier performance measure. <br /> Performance Measures Anticipated Results <br /> The length of the average match for Community-Based mentoring will meet or exceed 28 <br /> 22 months. <br /> The length of the average match for School-Based matches will meet or exceed 18 13 <br /> months. <br /> The match closure rate will remain below 40%. 40 <br /> At least 60%of the children matched with Bigs will experience improved grades. 60 <br /> Certified b} Title: V Date: �t- <br /> ( rovider's Signature) <br />
