EXHIBIT"A" Scope of Services—FY 2014- 15 Outside Agency Performance Agreement Agency Name: Chapel Hill-Carrboro Ymca Program Name: Boomerang Funding Award: $5,000 Outline how the agency will spend Orange County's funding award. Expense Description Amount Compensation for Program Manager $5,000 Program Services For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes below,by June 30,2015. • Daily academic support through 1:1 tutoring, participation in a Learner Profile, completion of a substance use assessment if relevant, participation in life skills group activities, completion of a strengths assessment,participation in a Wrap Up meeting. • Boomerang will provide at least 6 weeks of once weekly "What's Next" workshops on topics related to post secondary education,job exploration,and life skills during the school year. • Community Service hours offered Tuesday-Thursday from 4-6pm, as well as 3 designated community service days, providing students the opportunity for civic engagement while completing school required or court ordered service hours. • Providing a cohort of identified youth with additional support extending beyond their suspension period, including school visits, and sustained contact via social media, monthly reunions and other afterschool programming at Boomerang. Anticipated Outcomes The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants within that measure's description or for an earlier performance measure.
