EXHIBIT"A" Scope of Services—FY 2014- 15 Outside Agency Performance Agreement Agency Name: Oe Enterprises,Inc. Program Name: Job Development Funding Award: $45,100 Outline how the agency will spend Orange County's funding award. Expense Description Amount Funding award will be used to support personnel costs associated with the provision of $45,100 vocational placement,training, and community integration supports for students and adults who have disabilties. Program Services For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes below,by June 30,2015. • Employment related services provided to students includes include job placement, Job Club,job shadowing experiences, career explorations/counseling, assessments, assistance with volunteer experiences,and developing work portfolios. • Employment related services provided to adults includes self-determination training, vocational assessment, pre-vocational training,job placement services, intensive on-the-job training, and job follow up services as needed. • Anticipated Outcomes The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants within that measure's description or for an earlier performance measure.
