EXHIBIT"A" <br /> Scope of Services—FY 2014- 15 <br /> Outside Agency Performance Agreement <br /> Agency Name: Planned Parenthood Of Central North Carolina <br /> Program Name: ACA Enrollment And Sex Education Programs <br /> Funding Award: $20,000 <br /> Outline how the agency will spend Orange County's funding award. <br /> Expense Description Amount <br /> 50%of Community Educator and 5%of Director of Education,plus benefits (25%) 20,000 <br /> Program Services <br /> For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the <br /> contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes <br /> below,by June 30,2015. <br /> • Becoming a Responsible Teen (BART), a science-based, comprehensive curriculum designed to <br /> increase knowledge and skill-based protective factors about contraception use a <br /> • Smart Girls, a science-based, intensive adolescent pregnancy prevention program for middle <br /> school girls designed to build self-esteem and health decision making. <br /> • Alumni group for graduates of our teen education programs. <br /> • Parent workshops to improve parents' confidence in talking to their children about sex . <br /> • Workshops for young women(aged 19—25) about family planning and pregnancy prevention. <br /> • ACA enrollment assistance <br /> Anticipated Outcomes <br /> The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange <br /> County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants <br /> within that measure's description or for an earlier performance measure. <br /> Performance Measures Anticipated <br /> Results <br /> #of high school teens trained using BART program 15 <br /> #of contacts made by BART participants as teen educators 300 <br /> #of middle school girls(ages 10— 14)trained using Smart Girls curriculum 10 <br /> #of program alumni group that participate in annual student-led service-learning project. 8 <br /> #of parents of middle and high school students that participate in parent workshops. 15 <br /> #of women of reproductive age that participate in birth control workshops 15 <br /> #of patients during enrollment period that receive open enrollment information 350 <br /> #of patients during enrollment period that sign a Medicaid expansion card 140 <br />
