EXHIBIT A <br /> PROGRAM PLAN <br /> ORANGE COUNTY COMMUNITY-BASED PUBLIC HEALTH INITIATIVE <br /> (CBPHI) <br /> FISCAL YEAR OCTOBER 1994--SEPTEMBER 1995 <br /> Goad. The overall goal of this project is to improve the health <br /> status of African-American members in the Efland, Perry Hills, <br /> and Cedar Grove communities of Orange County. The overall <br /> efforts of the community-based project will be to strengthen <br /> the capacity of the communities in identifying, assessing, and <br /> addressing their own unique health-related problems. <br /> Objectives: <br /> 1. By the end of the fiscal year, the Orange County CBPHI staff <br /> will have identified and trained ten natural helpers from the <br /> community, using the Community Health Advocacy Program (CHAP) <br /> model . <br /> 2. By the first of January 1995, the Orange County CBPHI staff <br /> will have hired a part-time Teens in Power coordinator to <br /> coordinate services for a core group of at least twenty teens <br /> from the targeted communities. (Coordinated services will <br /> include up-ward mobility in health careers, health education, <br /> programs for teens, revision of Community Voices for teens, <br /> and recreational activities and cultural programming for <br /> teens. ) <br /> 3. During the fiscal year 1994-95, the Orange County CBPHI staff <br /> will continue meeting with the established community <br /> individuals trained in Community Voices and potential Voices <br /> candidates, to ensure the on-going development of community <br /> leadership skill implementation. J <br /> 4. To ensure a health careers component for 1994-95, the Orange <br /> County CBPHI staff will meet monthly with Wake Area Health <br /> Services staff to plan and implement educational programming <br /> for the teen population. <br /> 5. During the fiscal year 1994-95, the Orange County CBPHI staff <br /> will enhance project publicity in the community and solicit <br /> involvement from community residents: <br /> a. Community Newsletter -- to be developed <br /> b. Community Forum -- date TBA on or before 1-31-95 <br /> 6. During the fiscal year 1994-95, the Orange County CBPHI staff <br /> will continue meeting with established community organizations <br /> to ensure community empowerment. (Organizations to include <br /> United Voices of Cedar Grove, the Efland-Cheeks organized <br /> group [name TBAI , Northern Orange Low-Income Policy Advisory <br /> Council , and the Efl and-Cheeks/Cedar Grove Family Resource <br /> Center Steering Committee. ) <br />
