EXHIBIT"A" Scope of Services—FY 2014- 15 Outside Agency Performance Agreement Agency Name: Inter-Faith Council For Social Service Program Name: Food And Residential Programs Funding Award: $41,000 Outline how the agency will spend Orange County's funding award. Expense Description Amount Salaries for pantry director and kitchen manager $41,000 Program Services For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes below,by June 30,2015. • The Food Pantry will distribute monthly grocery allotments of essential food items to over 4,200 eligible members; quantities are based on the number of people in the household. • The Community Kitchen and HomeStart will serve hot meals three times daily to homeless residents and anyone who is hungry. • Residential Programs will be safe and clean locations that will provide emergency and transitional housing, health care, comprehensive on-site support services and a continuum of services to participating residents. Anticipated Outcomes The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Oran¢e County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants within that measure's description or for an earlier performance measure.
