Keep in mind that documentation of your good faith efforts is very important. Please note <br /> that per the Bid Documents that failure to comply or properly document good faith efforts <br /> is grounds for rejection of the contractor's bid. <br /> 10. With the bid submittal, you must submit the Identification Form and either Affidavit A (Listing of <br /> good faith efforts) or Affidavit B (Intent To Perform All Work). These documents are located in <br /> the Bid Documents. Contractors receive ten (10) points on the Good Faith Efforts form for <br /> attending this pre-bid meeting. <br /> 11. Affidavit C (Portion of Work to Be Performed By Minority Finns) or D (Good Faith Efforts)must <br /> be submitted by the low bidder within 72 hours of being notified as the low bidder, and other DBE <br /> documentation must be submitted within ten(10)days. <br /> 12. The CDBG requirements mandate that the Davis-Bacon Act be followed. Contractors are referred <br /> 1 to the Contract Documents for the full Davis-Bacon text and prevailing wage determinations. <br /> 13. "Buy American"provisions are not required for this project. <br /> 14. Mr. Chapman stated that all Bidders should visit the project site to thoroughly familiarize <br /> themselves with the work area. <br /> 15. An NCDOT Encroachment agreement was issued for the project, a copy of which is included in <br /> bid documents. <br /> 16. Mr. Chapman stated that all bidders shall include E-Verify affidavits with their bid. Applicable <br /> forms are included in the bid documents. <br /> 17. Mr. Chapman stated that the line item for mobilization in the Bid will only be paid once regardless <br /> of whether the Contractor moves on-site, completes part of the infrastructure, moves off the site, <br /> and later moves back on-site to complete the remaining infrastructure. <br /> 18. Questions: The following questions were asked at the meeting and then either answered or <br /> deferred to a written response: <br /> A. Is the contractor allowed to wait until near the 120 day mark to begin the project? <br /> Answer:Mr. Chapman stated that as long as the Contractor finishes the work by the end of <br /> the contract time, they can establish whatever schedule and staging they choose. <br /> B. What is Mebane's involvement? <br /> Answer: Mr. Chapman stated that Mebane's details and specifications are used for the <br /> project. He further indicated that while Orange County is constructing the project, <br /> Mebane will own and maintain the water and sewer infrastructure upon completion. <br /> C. Who will inspect the project, McGill or Mebane? <br /> Answer: Mr. Chapman stated that McGill would have periodic observation on the project, <br /> as well as staffftom Orange County and Mebane. Mr. Lindley stated that Mebane will be <br /> present for all testing, and should be contacted by the Contractor to schedule testing, in <br /> addition to McGill. <br />
