11. Excavation material shall not be laced on the pavement. Drainage structures shall not be ' <br /> P p g <br /> blocked with excavation material. <br /> 12. The encroaching party shall comply with all applicable federal, state and local environmental 1 <br /> regulations, and shall obtain all necessary federal, state and local environmental permits, <br /> including but not limited to,those related to sediment control,storm water,wetland, streams, <br /> endangered species,and historical sites. ' <br /> 13. All excavation in the roadway shall be backfilled and paved or covered with temporary steel <br /> plating. When steel plating is used the excavation shall be backfilled and/or shored to 1 <br /> accommodate anticipated traffic loads. A reflectorized bump warning sign (WB-1)must be <br /> installed in advance of the plate. All steel plating shall be pinned in at each corner with a <br /> smooth headed pin that does not protrude above the plate more than 0.5 inches and must 1 <br /> extend into the paved surface at least 3 inches. The plate must be extended at least 12 inches <br /> beyond the edge of the excavation in all directions and ramped with asphalt at least 6 inches <br /> wide. 1 <br /> 14. Trenching, bore pits, and other excavations with the safe recovery area shall not be left open <br /> so as to expose motor vehicle or pedestrian traffic to unsafe conditions. The contractor shall 1 <br /> comply with all OSHA requirements and provide a competent person on site to supervise <br /> excavations at all times. <br /> 15. All suitable backfill material shall be compacted to 95%density in the shoulder areas, 100% 1 <br /> density under pavement areas and in accordance with AASHTO T99 as modified by the <br /> North Carolina Department of Transportation. Note if contractor starts before weather <br /> conditions improve, then the existing material removed shall be replaced with suitable 1 <br /> material as approved by the District Engineer so that proper compaction can be obtained. <br /> Certified compaction test results are to be submitted to the District Engineer upon request. <br /> 16. Deep excavation adjacent to open travel lanes which exceeds a one to one (1:1) influence 1 <br /> slope shall be supported by a protective support system designed by a professional engineer <br /> to adequately carry all dead and live loads or backfilled prior to returning traffic to normal <br /> patterns. <br /> 17. All disturbed areas shall be repaired with the following rate of seed, fertilizer and limestone 1 <br /> in pounds per acre,and the areas properly mulched and tacked: <br /> 25#Reliant Hard Fescue 1 <br /> 100#Alta or Kentucky 31 Tall Fescue <br /> 25#Kenblue Kentucky Bluegrass <br /> 1000# 5-10-10 Fertilizer <br /> 4000#Limestone <br /> 1 <br /> 1 <br /> 1 <br /> 1 <br />
