EXHIBIT A <br /> SERVICE ELECTION SCHEDULE <br /> Services selected by Participant as part of this agreement are defined in the table below. <br /> Service Definition <br /> HIE Connectivity Services NC HIE will connect Participant to the HIE network and allow for the exchange <br /> of healthcare information between authorized Participants.The HIE <br /> Connectivity Service will provide Participants the ability to <br /> a) publish information to other Participants, <br /> b) the ability for other Participants to publish information to Participant, <br /> and <br /> c) the ability for Participant to query the HIE network for information <br /> The HIE network will provide the following services as part of supporting the <br /> exchange of information: <br /> i. Enterprise Master Patient Index <br /> ii. Provider Directory <br /> iii. Privacy and Consent Module <br /> iv. Integration Bus supporting HL7,CCD,and other industry standards <br /> V. Normalization and Transformation Module <br /> vi. Auditing and Compliance Function <br /> Patient Record Network NC HIE will make available to Authorized Users of Participant access to the <br /> Patient Record Network portal.This portal can be used to view the longitudinal <br /> patient record,set alerts and notifications,and access other HIE services <br /> Direct Secure Messaging NC HIE will provision Authorized Users of Participant with an ONC compliant <br /> Direct messaging environment. Each Authorized User will be provided access <br /> to a web-based messaging environment with a unique address assigned in the <br /> NC HIE Direct domain (facilitycode.firstname.lastname direct.nchie.oCg). <br /> Users will have the ability to append files containing Protected Health <br /> Information to their Direct messages. <br /> Lab Distribution Program NC HIE will provide a common lab interface to Participants through which they <br /> can receive lab results from lab companies that are participating in this <br /> program. Submitting lab orders is not yet available. <br /> Hospital Lab Integration Routing of hospital lab results through common HIE interface to Participants. <br /> Routing of lab orders is not yet available. <br /> State Connectivity 1. Bi-directional routing of patient immunization information through a <br /> Services common HIE interface. <br /> 1. NOR <br /> CCNC Disease Registry Exchange of Patient Health Information (PHI)with CCNC Disease Registry for <br /> Diabetes <br /> Training services NC HIE will offer training services for the initial on-boarding for Portal use and <br /> Opt-out. <br /> Integration Services NC HIE will offer integration services for connectivity to the HIE Network for <br /> the services selected by the Participant <br />
