Contract#68-5000 <br /> Inter-Faith Council for Social Service,Inc. <br /> B. Contractor will be responsible for all other information required for federal reporting. <br /> Contractor will be notified of these reporting requirements as federal guidance is provided. <br /> C. Contractor will be responsible for submission of semi-annual reports to the State. <br /> Documentation: CHIN reports,Annual reports, Semi-Annual reports. <br /> IV. Reimbursement <br /> The Contractor shall submit to Orange County Department of Social Services by the 10th of each <br /> month, a monthly reimbursement request for services rendered the previous month. Contractor will <br /> submit reimbursement requests using forms supplied by the State with required documentation of <br /> expenses (copies of invoices/bills/receipts)and proof of payment(copies of ledgers or copies of <br /> checks used to pay expenses). <br /> Contractor will maintain additional required documentation in household (participant) files and in <br /> the CHIN system. Reimbursement requests will be verified against documentation of expenses and <br /> proof of payments and may be verified against data in CHIN before payment in made. <br /> Documentation: Reimbursement forms,reimbursement documentation, CHIN reports,participant <br /> files,timesheets or documentation of indirect cost rates,cost allocation plans,copies of paid <br /> invoices/bills/receipts and copies of ledgers or checks used to pay invoices/bills/receipts. <br /> OTHER FEDERAL REQUIREMENTS <br /> I. Homeless Participation <br /> A homeless or formerly homeless person must participate to the fullest extent possible in the <br /> development of ESG-funded programs and policies set forth by the Department of Housing and <br /> Urban Development(HUD). <br /> Additionally,to the maximum extent practicable,the Contractor will involve,through employment, <br /> volunteer services, or otherwise,homeless individuals and families in maintaining and operating <br /> facilities assisted under ESG, and in providing services for occupants of facilities assisted under <br /> ESG. <br /> II. Accessibility of Spaces,Services,and Activities <br /> Contractor shall operate each existing program or activity receiving federal financial assistance so <br /> that the program or activity,when viewed in its entirety, is equally accessible to and usable by <br /> individuals with disabilities as by persons without disabilities. <br /> III. Property Standards <br /> All dwelling units that are a part of an ESG-funded activity shall meet minimum habitability <br /> standards set by HUD. Shelter facilities are also required to meet state or local government safety <br /> and sanitation standards, as applicable, including energy-efficient appliances and materials. <br /> IV. Lead-based Paint <br /> All shelters assisted under the ESG program and all housing occupied by program participants must <br /> adhere to the requirements of the Lead-Based Paint Poisoning Prevention Act(42 U.S.C. 4821- <br /> Outcomes (06104) Page 4 of 9 <br />
