54 <br />ROLL -OFF DRIVER - PART 2 <br />Initial & Date <br />• Controlling Front Tire Blow Outs <br />• Proper completion of paperwork (VCR's, route sheets, time cards) <br />• Road Observations on a Route <br />• Orientation of Local Landfill, Transfer Station, etc. <br />• Practice loading and unloading various types of containers in the yard. <br />• Have the new employee cover all aspects of the operation including disposal <br />' <br />• Have the new employee, under supervision, pull containers on a route. <br />• Review all paperwork to ensure the new employee is filling it out correctly <br />• Complete a Driver Training Checklist, for Roll -Off drivers <br />Meet with the employee each day; document and discuss any issues or concerns. <br />o Perform the final evaluation with the new employee <br />ROLL -OFF DRIVER - PART 3 <br />Initial & Date <br />o Assign the employee to a specific route <br />F i <br />o Complete at least one Observation Report per week, for first three weeks <br />i <br />o Assess the employees performance weekly; document and discuss any issues <br />I <br />o Ride with the employee after 30 days, compete a Driver Training <br />Checklist for Roll -Off drivers and evaluate the employees overall performance. <br />o Meet with the employee on the 51 st day; document any issues or concerns. <br />o Conduct a final evaluation on the 82nd day using performance appraisal forms. <br />}{ <br />i <br />€ have received training in the topics listed above on the dates indicated. l have had the opportunity to ask <br />questions and received answers on the contents of this training presented by the Company.
