The purpose <br />is to check the understanding of proper operating procedures for the type of vehicle to be assigned and <br />shall be typical of the anticipated routes and operating environment. <br />ESTABLISH AND MAINTAIN FILES <br />Each personnel file must include the following completed documents as requested from the various <br />departments. <br />Personnel file required forms <br />o Application <br />o Employment Eligibility Verification (Form 1-9) <br />o Other required forms (tax, enrollment, release...) <br />Driver Qualification File Required Forms (in addition to Application) <br />• DOT Physical Examination Form <br />• Inquiry of past employers <br />• Violation and Record Review <br />• Record of Road Test <br />• Certification of Road Test <br />• Notice to Drivers and Certificate of Compliance <br />• Controlled Substance Test Results (Chain of Custody) <br />• Notice of Disqualification <br />• Annual Review of Driving Record <br />• Pre - placement Urinalysis Notification (release) <br />• Copy of Drivers License <br />• MVR (current) <br />Safety Required Information <br />• Results of pre - placement physical examination (DOT file only) <br />• Commendations and written warning letters (safety related) <br />• Copies of accident report <br />• Copies of workers compensation claims and any additional claim information <br />• Acknowledgment of training received <br />• Acknowledgment of safety rules received <br />• Attendance of safety meetings <br />
