43 <br />POTENTIAL DRIVERSIOPERATORS <br />The section includes all information necessary to meet the U.S. Department of Transportation (DOT) <br />requirements for employment as a driver. The term driver is used to include any person driving; a vehicle <br />with the gross weight exceeding 10.001 Pounds; any company insured vehicle, and any employee who <br />drives as a requirement of their job. Designated maintenance personnel assigned to duties such as road <br />test (troubleshooting) and road calls must possess a CDL prior to operating a company vehicle on the <br />public highway and within six months of employment. The "basic" hiring requirements previously <br />mentioned are also prerequisites for driver positions and will be a part of the Driver Qualification File, <br />Standard Driver files are available through the Waste Industries website. Driver Qualification Files must be <br />organized and maintained at the branch for as long as p of all CDL holders until 3 years after, <br />For the purpose of consistency in hiring, equipment operators will undergo the same process with the <br />exception of a DOT physical and driver qualification files. Although not required for equipment operators, <br />the Record of Violations is considered a verification of the last twelve months of operating experience and <br />is a simple method of cross - referencing MVR's. <br />Employees hired to drive company commercial motor vehicles must be a minimum 21 years of age. <br />Current non - driving employees will not be promoted to a driving position until they meet this age criterion. <br />Physical Examinations <br />Each driver shall be examined to meet the criteria listed by the DOT <br />o The DOT physical examination form and the medical examiners certificate card shall be given to the <br />driver to be presented to the physician at the time of the examination. <br />• The physician will complete the DOT form and medical card and return it to Waste Industries. Should <br />the driver fail the examination, the physician will retain the test information and provide the results via <br />telephone to the branch. C <br />• The completed physician examination form shall be retained in the drivers file and the driver shall <br />carry the medical examiner's certificate card at all times while driving. <br />Inquiry to Past Employers <br />Each employer that the driver was employed with or contracted to drive for within the past 10 years shall <br />be listed on the application. All employers within the past three years shall be contacted either by phone or <br />in writing. Each inquiry sent by mail shall be copied and filed. Telephone interviews shall be appropriately <br />documented. <br />Violation and Record Review <br />Each potential driver will complete this form identifying convictions within the past 12 months occurring in <br />any type of vehicle or motorcycle. (Parking tickets do not apply.) <br />Motor Vehicle Report (MVR) <br />MVR's are obtained before an individual is offered employment in any driving capacity. (This includes <br />equipment operators and maintenance personnel). MVR's will ensure that the potential employee's license <br />is valid with the proper license class when applicable.
