42 <br />Section 2,10 HIRING GUIDELINES (SAFETY /DOT) <br />PURPOSE <br />Proper applicant screening is a vital step in the selection process of any employee. This is especially true <br />for fleet operations where driver and equipment operator selection is the cornerstone of loss control. <br />APPLICATION <br />The following guidelines are intended to provide minimum acceptable standards for the hiring of <br />employees operating in safety sensitive positions. Safety sensitive activities, for the purpose of this <br />procedure, are job functions associated with the operation of a commercial motor vehicle, heavy <br />equipment and /or where historically the company has experienced a higher number of losses from <br />workers compensation claims or accidents. These procedures in no way supersede or negate any <br />established personnel hiring procedures. <br />ALL PROSPECTIVE EMPLOYEES <br />All prospective employees in safety sensitive positions will be processed using the following tools: <br />Application and Employment Eligibility Verification (Form l•9) <br />o Each applicant will complete and sign a company approved employment application prior to hire <br />o The Eligibility Verification (Form 1-9) will be completed within three days of employment <br />Authorization & Release <br />Each applicant's signature should be obtained on a standard authorization and release form: <br />• Enables the necessary background investigation, records review and alcohol /drug history review <br />• Permits updating for required information, as necessary <br />Interview <br />The interviewer should attempt to ascertain employment experience, knowledge of DOT regulations and <br />traffic laws (driver applicants), and opinions regarding past employment and safety experience. Specific <br />information will be noted and later reviewed and investigated to detect discrepancies or falsification. <br />Pre - Placement, Post -Offer Medical Examinations <br />After a driver is offered employment contingent upon the results of a medical examination, make <br />arrangements for a physical exam at a designated medical facility. Medical examinations are required by <br />FMCSR every 24 months. It verifies an individual is DOT qualified as well as capable of meeting the <br />physical demands associated with the essential job functions. In order to properly make that <br />determination, the medical facility must be acquainted with DOT medical qualifications and /or be provided <br />with the appropriate regulations and an accurate job description. <br />NOTE: DOT requires verification that a driver has a current medical card. Wl considers current to <br />be within the past six months and driver must possess a copy of the physical examination (long <br />form not card).
