31 <br />Safety Regulations and Rules <br />Branch specific safety rules should be established and prepared in typed or printed form for <br />distribution to supervision and employees. Company rules and regulations can be used in total to <br />satisfy this requirement. Also, procedures should be established for the safe operation and <br />handling of specific equipment, process jobs, or machines, such as balers, welders, jacks, lifts, <br />steam jenny, presses, tire machines, compactors, etc.. <br />0 General Inspections <br />The achievement of safe working conditions requires the elimination of both unsafe physical <br />conditions and unsafe practices. A good inspection should include following: <br />• Regular inspection by supervisors of areas for which they are responsible <br />• Periodic and regular unannounced inspections by the General /Branch Manager <br />• Assignment of specific responsibility for the maintenance of good working conditions <br />• Preventative Maintenance - All branches will have periodic inspections. The frequency of <br />these inspections will be once per month at a minimum. Each branch shall use the company <br />developed checklist or one containing all those items at a minimum. <br />o Safety Lanes —All branches utilizing Commercial motor vehicles (CMV) will establish and <br />maintain a vehicle safety inspection process, to occur at the end of a vehicles shift, where the <br />driver and maintenance personnel communicate directly regarding vehicle condition and <br />performance. This action does not eliminate the daily post -trip required by federal law. <br />Accident Investigations <br />An accident investigation has two immediate purposes; (1) to get all of the relevant facts, and (2) <br />to develop recommendations to prevent a repeat. Use established procedures for accident <br />investigations, time limits for reports, and means for implementing recommendations to prevent <br />reoccurrence. At a minimum, each investigation should include: <br />• Separate Driver /Operator and Passenger statements and a written description of accident. <br />• Vehicle and operator /driver information for all involved parties including witness statements <br />• Accident diagram including vehicle direction, traffic controls and other pertinent information <br />• Be specific regarding the accident location, weather conditions and road surfaces <br />• Communication to Health, Safety and Risk Management within required time frames <br />Refer to Section 2,60, "Accident Reporting and Investigations ", for complete details regarding all <br />procedures associated with accident investigations and/or reporting. <br />0 Maintenance of Accident Records and Statistics <br />An accident register (list) and all backup documents such as comprehensive accident reports <br />must be kept by each branch for a minimum of one year (Branch Compliance Folder in WISER), <br />to analyze past experience, meet legal requirements, and guide future planning. <br />Pli'avie industria.s i 1!01/11 Rsd ssc. - . <br />
