13 <br /> TOWARD . . . <br /> A RE-DESIGNED SYSTEM <br /> The following recommendations are appropriate for all County funded <br /> human services agencies, including nonprofits, and for other human <br /> services funders, such as United Way and local municipalities. <br /> OUTCOMES AND EVALUATION <br /> • Approve the vision, mission, guidelines, outcomes criteria, goals and <br /> recommendations presented in this report. <br /> • Authorize a community consensus building process to develop outcome <br /> expectations for the essential elements of well-being. <br /> • Fund, create and put into place, at the beginning of the process, an <br /> outcomes-based evaluation system that includes: <br /> 1) training and resources for framing feasible outcome statements <br /> and accurate benchmarks, <br /> 2) equipment and training necessary to track and evaluate outcomes, <br /> 3) outside technical support to give comments on the evaluation <br /> process, and <br /> 4) an annual review and revisions of the human services vision, <br /> mission, guidelines, goals, expected outcomes and actual outcomes. <br /> USER-FRIENDLY SERVICES <br /> • Involve individuals and families directly in making decisions relating to <br /> the services they receive and in evaluating the delivery of services. <br /> • Involve staff, program participants and the community-at-large in <br /> planning and evaluating programs and services. <br /> • Create a unified management information system for services <br /> recipeints that uses a single intake form, assigns a single identification <br /> number and accommodates multiple entry points. <br /> • Institute a 1 year follow-up of all people who lose public assistance <br /> eligibility to determine their ability to provide for themselves and to <br /> identify any increased cost to the public due to loss of services. <br />
