PROGRAM PLAN EXHIBIT A <br /> ORANGE COUNTY COALITIO, <br /> NC CONDAUNITY-BASED PUBLIC HEALTH INrrIATIVE <br /> PROGRAM YEAR October, 1995 - September, 1996 <br /> GOAL: <br /> The goal of the Orange County Coalition is to improve the health status of selected <br /> communitics, and to narrow the gap in health outcomes between majority and minority <br /> populations in communities such as Efland, Perry Hills(Cheeks) and Cedar Grovc. The <br /> Coalition's primary focus will be: 1) to strengthen the capacity of the communities in <br /> identifying, assessing and addressing their own unique health related problems; and 2) to <br /> institutionalize the community-based public health concept and to strengthen the <br /> collaborative efforts of communities, schools, and local governmental organizations and <br /> agencies. <br /> OBJECTIVES: <br /> A. Improve the health status of minority communities in selected communities by <br /> addressing community identified health issues of chronic diseases and basic child health services. <br /> STRATEGIES: <br /> ♦ Work with a core group of community residents and leaders to develop a diabetes <br /> coalition that would establish and implement a"Diabetes Today"program for the <br /> Efland-Cheeks/Cedar Grove community. <br /> ♦ Work with a core group of community residents and leaders to develop and <br /> implement a community self-health diagnosis. Explore combining the <br /> community health survey of the targeted communities with the community <br /> diagnosis proposed by the Healthy Carolinians' Task Force. <br /> ♦ Sponsor a community health forum to increase the community's awareness of its <br /> health status according to information gathered through the survey. <br /> ♦ Enlist ministers it the targeted co:^.r"��WllY�rs to participate in the CBPHI's <br /> Ministry of Health Initiative <br /> B. Enhance the capacity and potential of residents and community-based organizations <br /> by nurturing their leadership capabilities in order to address their own issues and concerns. <br /> STRATEGIES: <br /> ♦ Work with the leaders and members of United Voices of Northern Orange County <br /> to develop annual work plan for 1996-97. <br /> ♦ Continue work with United Voices to develop and implement plans of action for <br /> the following: the Efland-Cheeks School and Community Paris, a Efland-Cheeks <br /> ® a3 <br />
