May 22, 1995 EXHIBIT A <br /> COMMUNITY BASED PUBLIC HEALTH INITIATIVE <br /> MINI-GRANT PROPOSAL <br /> ORANGE COUNTY HEALTH DEPARTMENT, NORTH CAROLINA CONSORTIUM <br /> OVERALL GOAL <br /> To provide a training program for health department staff <br /> which would enable them to develop the knowledge, attitudes <br /> and skills to carry out the diagnosis of health problems as <br /> well as the planning and delivery of public health services <br /> in partnership with the community through a community based <br /> public health (CBPH) approach as defined in the Kellogg _ <br /> program. <br /> Component #1 <br /> The Orange County Health Department has joined with the other <br /> three public health practice partners to participate in a <br /> Shared Leadership and Model Development training program. <br /> The proposal for this training will be submitted separately. <br /> Component #2 <br /> Additional training will be provided to a broader <br /> representation of health department staff than can <br /> participate in the combined training. The training program <br /> would be a series of sessions with practical, hands-on <br /> experiences that would take participants through a process <br /> rather than a one-time effort. Emphasis would be not only on <br /> increasing knowledge but also on changing attitudes and <br /> developing the skills necessary to operate through the CBPH <br /> approach. Discussion would include how the CBPH approach <br /> relates to the mission and roles of the health department and <br /> how the approach could be used to carry out the overall <br /> responsibilities. of the health department. Some sessions <br /> might involve community members and representatives of other <br /> community agencies the department works with to bring about <br /> change in the community. <br /> Component #3 <br /> Complete a diagnosis of community health problems in Orange <br /> County utilizing not only secondary data analysis but <br /> also quality input from the community-at-large. Strategies <br /> used to obtain this input would include surveys, focus <br /> groups, community forums, etc. <br />
