5. Testing Administration <br /> Orange County will provide (Cedar Grove Day Care, Cedar Grove Head Start or OPC <br /> Mental Health) with a current listing of approved testing locations for drug and alcohol <br /> testing and required Federal Drug Testing Custody and Control forms. <br /> Orange County will notify the designated (Cedar Grove Day Care, Cedar Grove Head <br /> Start or OPC Mental Health) representative when an employee has been identified for a <br /> random drug or alcohol test. (Cedar Grove Day Care, Cedar Grove Head Start and OPC <br /> Mental Health)will arrange for the employee to report to an approved testing location <br /> within the period specified by Orange County. <br /> (Cedar Grove Day Care, Cedar Grove Head Start or OPC Mental Health)will be <br /> responsible for identifying when conditions for drug and alcohol testing exist and will <br /> arrange for an necessary testing related to reasonable suspicion, pre-employment, post- <br /> accident and return-to-duty and follow-up as required by the Federal Department of <br /> Transportation. <br /> (Cedar Grove Day Care, Cedar Grove Head Start or OPC Mental Health)will provide <br /> Orange County Personnel Department with a Drug and Alcohol Testing Consortium Data <br /> Change form each time there is a change in status for an employee subject to the federal <br /> drug and alcohol testing requirements. (This includes notification of new hires and <br /> terminations.) <br /> 6. Testiniz Results <br /> Results of drug and alcohol tests will be released only to the designated agency <br /> representative upon receipt of the testing results by Orange County. <br /> 7.
