Attachment 3 5 <br />A <br />AMERICAN TOWER" <br />C O R P O R A T I O N <br />NEW SITE DEVELOPMENT EXECUTED AGREEMENT INSTRUCTIONS <br />In order to ensure the prompt execution and processing of all agreements, American Tower <br />respectfully requests that the instructions below are followed when preparing to return executed <br />agreements. <br />1. Prior to returning to American Tower, please sign and date the signature page of all copies of <br />the Lease and Memorandum of Lease. <br />2. Ensure that all necessary witness signatures have been obtained (if witnesses are required). <br />3. Ensure that all applicable notary fields are completed and stamped with the notary's stamp. <br />4. Please complete a W -9, Payment Authorization Form, and if applicable, a mortgage <br />information form. <br />5. American Tower will retain two (2) originals of each document. Please print as many copies <br />as you plan to retain, along with two originals for American Tower. <br />6. Please print all signed documents on one -sided paper. <br />7.
