Exhibit A—Scope of Services—Fiscal Year 2013-14 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $ 20,000 <br /> Agency: Housing for New Hope <br /> Agency Mission: The mission of Housing for New Hope is to prevent and end homelessness by providing <br /> increased access to Healthcare, Integrated Services, and Housing. <br /> Program: The PATH outreach team heads the effort in assisting the unsheltered, homeless <br /> individuals within the Orange County area. In addition to housing assistance,the team <br /> assists with employment, mainstream benefits and connecting to primary healthcare. <br /> Purpose: To engage homeless individuals where they reside while connecting them with services <br /> and permanent housing <br /> Outcomes: Overall,the desired outcome is to decrease the unsheltered homeless on the streets of <br /> Orange County. We plan to accomplish the following: Outreach at least 145 individuals, <br /> enroll 60 into services which include housing counseling and medical navigation.
