Exhibit A—Scope of Services—Fiscal Year 2013-14 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $ 5,000 <br /> Agency Community Empowerment Fund <br /> Agency Mission: To cultivate opportunities, assets and communities to support the alleviation of <br /> homelessness and poverty <br /> Program: CEF's Advocate Program aims to enable and sustain transitions out of homelessness. <br /> Through the time and energy of committed volunteer Advocates, the program provides <br /> homeless and near-homeless individuals with personalized support and one-on-one <br /> assistance throuqhout their recovery from acute economic crisis. <br /> Purpose: The purpose of the program is to assist individuals towards achieving: <br /> 1. Greater employment <br /> 2. Housing stabiility <br /> 3. Increased household assets <br /> 4. Stronger connections to available community resources and social support <br /> Outcomes: 1)CEF will increase our volunteer base to 150 active volunteers meeting regularly with CEF <br /> members <br /> 2)At least 100 members will gain greater employment <br /> 3) Fifty asset-poor households earning less than 30%AMI will achieve personal savings <br /> goals and move towards creating greater family wealth.
