Exhibit A—Scope of Services—Fiscal Year 2013-14 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $ 4,000 <br /> Agency: The Arc of Orange County <br /> Agency Mission: The Arc of Orange County was incorporated in November of 1979 as a 501©3 nonprofit <br /> organization. Entering into our 34th year of operation, The Arc of Orange County is proud <br /> of its mission to provide advocacy and services to nersons with develonmental disabilities© <br /> Program: Funding provided through this application will be used to provide to children (ages 3 and <br /> above)and adults with Intellectual and developmental disabilities residing in Orange <br /> County, North Carolina.The Programs and activities described in this application are <br /> Purpose: Through the use of our formalized volunteer services division and our direct care staff, The <br /> arc of Orange County links individuals with available and willing existing community <br /> resources and people and thus significantly assist persons with I/DD in achieving lives that <br /> they choose and in being viewed by the community as valued citizens with much to <br /> contribute. Subsequently,this leads to a lessoning of social isolation, increased quality of <br /> UP and n orPatPr laval of sPlf-ciiffiriPnry for thnsP invok/PrI Oiir C;ommiinity C:nnnartinns <br /> Outcomes: Community Inclusion Programs-Utilizing 50+volunteers,we assist individuals in becoming <br /> full participants in the community. Services will include daily/weekly social, vocational, <br /> recreational and educational activities in Community Settings. Regular communication with <br /> people receiving or coordinating serves and activities including emails,website information, <br /> written brochures, newsletters and regularly updated activities calendars are provided and <br /> updated weekly. <br /> Community Connections-Our Community Connections Program links persons with <br /> disabilities with volunteers.
