36 <br /> Instructions for LOCOS Access <br /> The information provides you with the required steps that <br /> must be taken by designated Program/Field Office staff to access <br /> the Line of Credit Control System (LOCOS) . Staff will need both <br /> Query and Field Office Administration capabilities in LOCCS . The <br /> required actions are: <br /> 1. A Form HUD-27054-A, LOCCS Access Authorization Security <br /> Form must be completed and signed. The form must also <br /> be signed by the appropriate Authorizing Official . The <br /> Authorizing Official must also complete a Form HUD- <br /> 27054-A. After completion and signature, the form <br /> should be sent to the Regional Personnel Office, who <br /> will certify completion. <br /> 2 . A Departmental ADP Policy Statement must be reviewed <br /> and signed by the designated Field Office staff . <br /> 3 . An SF-85P (Questionnaire for Public Trust Positions - <br /> original and 2 copies) must be completed and signed. <br /> The form should then be sent to the Regional Personnel <br /> Office, who will certify completion of the form. <br /> 4 . Forms SF-171 (Application for Employment - Original and <br /> one copy) , and SF-87 (Fingerprint Form - two copies) <br /> must also be completed and signed. These forms should <br /> be sent to the Regional Personnel Office, who will <br /> certify for completion. <br /> S .
