The TIN is the payee' s Employer <br /> Identification Number. This is a nine digit number <br /> separated by a hyphen after the first two digits; <br /> e.g. , 00-0000000 . <br /> PAYEE/JOINT PAY CERTIFICATION: Signature of an official of the <br /> organization who has authority to direct HUD to send <br /> funds to the designated payee as shown in Block A. <br /> BLOCK D: Insert and "X" for either checking or saving. <br /> BLOCK E: Insert the financial institution account number of the <br /> payee shown in Block A.
