ORANGE COUNTY <br /> ORANGE PUBLIC TRANSPORTATION <br /> DRIVER AGREEMENT <br /> This Agreement is entered into this first day of July, 1997 by and between the County of <br /> Orange on behalf of the Department on Aging's Orange Public Transportation service, <br /> hereinafter referred to as "Orange County" and Orange-Person-Chatham Mental Health <br /> Center, hereinafter referred to as "OPC ". <br /> The purpose of this Agreement is to outline the conditions under which Orange County <br /> and OPC will cooperatively provide human services transportation through the Orange <br /> Public Transportation service. <br /> SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY <br /> 1. To provide one certified driver for courier service among OPC sites and daily pick-up <br /> and take-home of Adult Day Treatment clients. This driver will be supervised by <br /> Orange County. <br /> 2. To provide back-up drivers in case the assigned driver is not available. <br /> 3. To employ a Transportation Supervisor to handle any problems and to assure that <br /> drivers are available. <br /> 4. To comply with OPC transportation policies and procedures as well as state <br /> standards. <br /> 5. To comply with all applicable federal regulations governing workplace anti-drug and <br /> alcohol programs in the transit industry. The Federal Transit Administration (FTA) of <br /> the U.S. Department of Transportation has published 49 CFR Part 653 and Part 654, <br /> as amended, that mandate urine drug testing and breath alcohol testing for safety <br /> sensitive positions and prohibits performance of safety sensitive functions when there <br /> is a positive test result. <br /> SPECIFIC RESPONSIBILITIES OF OPC <br /> 1. To reimburse Orange County at the rate of$12.25 per hour for seven hours per <br /> workday of Orange County's operation within 30 days after billing. <br /> 2. In the event that OPC requires specialized training beyond driver's current level, it <br /> shall be their responsibility to indicate the type and level of training and to provide the <br /> prescribed training without cost to Orange County. <br /> 3. To provide the Transportation Supervisor written notification of any transportation <br /> problems or unsatisfactory drivers' performance in order that corrective action can be <br /> taken. <br /> 4. To provide the Transportation Supervisor written notification of any changes to the <br /> fixed route at least three working days prior to the effective date. <br /> Page 1 of 2 <br />
