STATE OF NORTH CAROLINA UNCH#88 <br /> COUNTY OF ORANGE <br /> AGREEMENT BETWEEN <br /> THE UNIVERSITY OF NORTH CAROLINA HOSPITALS <br /> AND <br /> ORANGE COUNTY,NORTH CAROLINA <br /> THIS AGREEMENT, made effective this 1 st day of July, 1997, by and between <br /> Orange County (hereinafter referred to as the COUNTY) and The University of North <br /> Carolina Hospitals (hereinafter referred to as the HOSPITAL). <br /> WITNESSETH: <br /> WHEREAS, the HOSPITAL has agreed to pay certain compensation for said <br /> service and the parties desire to execute this Agreement to delineate their understanding of <br /> their agreement; <br /> NOW, THEREFORE, the parties hereby agree as follows: <br /> 1. The COUNTY agrees to make available to the HOSPITAL the services of one <br /> Social Worker I, one day (8 hours) per week at the Department of Obstetrics and <br /> Gynecology Clinic. <br /> 2. The HOSPITAL agrees to reimburse the COUNTY within 15 days of receipt of <br /> monthly billings for the salary and benefits involved in maintaining one Social Worker I at <br /> the HOSPITAL. The HOSPITAL also agrees to reimburse the COUNTY for 2 percent of <br /> the salary and benefits of a full-time supervisor. Salary and benefits for the Social Worker <br /> I and the supervisor include: base salary under State Personnel Regulations; FICA taxes, <br /> county government retirement, paid vacation, sick leave, or other leave under approved <br /> county plan, paid holidays as observed by county; paid petty leave of 14 hours per year; <br /> necessary travel, and per diem for job related activities. <br /> 3. Other supportive expenses provided by the HOSPITAL include: office space; <br /> parking space; office equipment; clerical support; and telephone service as required to <br /> adequately support the services provided by this Agreement. The HOSPITAL also agrees <br /> to pay a processing fee of$2.00 for each"out of Orange County" case presented. <br /> 4. The Social Worker I shall receive, process, and certify or deny applications for <br /> financial assistance under programs administered by OCDSS and originating in the <br /> HOSPITAL OB/GYN Clinic. This worker shall be assisted by the HOSPITAL staff in <br /> obtaining needed information on active cases eligible under the Department of Social <br /> Services Programs. Assignment of work to this Social Worker I will be the responsibility <br /> of a designated HOSPITAL staff member. <br />
