3 <br /> SUBGRANT NO. 5-52426-SG-800 <br /> between <br /> NORTH CAROLINA AGRICULTURAL & TECHNICAL STATE UNIVERSITY <br /> and <br /> CO -BASED PUBLIC HEALTH INITIATIVE <br /> This Agreement entered into by and between North Carolina Agricultural & Technical State <br /> University,Greensboro,North Carolina,hereinafter referred to as"GRANTOR",and Community- <br /> Based Public Health Initiative, Hillsborough, NC, hereafter referred to as "GRANTEE". <br /> VVITNESSETH THAT: <br /> WHEREAS,the GRANTOR has undertaken certain efforts for W Kellogg Foundation,under <br /> Prime Agreement No P0027656; and <br /> WHEREAS, the GRANTEE represents that it is ready, willing and able to undertake a portion <br /> of said efforts as hereinafter set forth; <br /> NOW THEREFORE, in consideration of the premises, as well as the obligations made and <br /> undertaken, the parties hereto mutually agree as follows: <br /> Article L STATEMENT OF WORK <br /> GRANTEE shall perform the effort set forth in the STATEMENT of WORK attached hereto and <br /> incorporated herein as Exhibit A. <br /> Article II. PERIOD OF PERFORMANCE <br /> The Agreement shall commence effective from date of acceptance. All work hereunder shall <br /> be completed on or before December 31, 1997. <br /> Article III. C:RANTFE CONTACT <br /> The contact person for this Agreement is Mr. Daniel B. Reimer, Director, Orange County <br /> Health Department.
