Because the <br /> Health Director has served as the fiscal agent for the Community-Based Public Health Initiative, <br /> he was named as the Grantee Contact for this agreement. <br /> Background: <br /> At the Commissioner's meeting of June 2, 1997 the Phase H Grant of the Community-Based <br /> Public Health Initiative was approved. This additional subgrant from NCA&T will support goals <br /> established by the CBPHI Coalition representing the United Voices of Northern Orange County, <br /> Orange County Cooperative Extension Service, the Health Department, the School of Public <br /> Health, Recreation and Parks Department and the Joint Orange Chatham Community Action <br /> Agency(JOCCA). The outcomes to be achieved by this subgrant include the following: <br /> 1. Conduct leadership development training with citizens from Efland Cheeks and <br /> Cedar Grove communities. <br /> 2. Apply for 501(c)3 status for both Community Voices groups. <br /> 3. Develop a youth entrepreneurship program in collaboration with the Cooperative <br /> Extension Service. <br />
