f. <br /> 3 <br /> 5. The Social Worker I shall work cooperatively with the HOSPITAL staff and <br /> the social work staff of any county Department of Social Services to make appropriate <br /> referrals of patients and family members with problems not related to eligibility <br /> determinations. <br /> 6. As an employee of Orange County, the Social Worker I shall be directly <br /> supervised by and accountable to the Orange County Department of Social Services. Due <br /> to the nature of this Agreement and the working relationship with the HOSPITAL, it is <br /> necessary that close contact be kept with the HOSPITAL administration and certain <br /> members of the HOSPITAL medical staff. In recognition of this factor, the Director of <br /> Fiscal Services of the HOSPITAL shall name a staff member to act as liaison between the <br /> Social Worker I, the departments of the HOSPITAL, and other staff personnel. Changes <br /> in duties and/or basis of financial participation may be made after prior written approval <br /> and agreement between the HOSPITAL, the Department of Obstetrics and Gynecology, <br /> and the COUNTY. Any changes in this Agreement to add personnel will require the <br /> approval of the Orange County Board of Social Services and County Commissioners. <br /> 7. This Agreement shall be for the term July 1, 1997 through June 30, 1998. <br /> 8. It is the intention of the parties that this Agreement be reviewed annually, as <br /> near July 1 st each year as possible, and may be terminated by either party upon 60 days <br /> written notice but preferably at the beginning of each fiscal year. <br /> 9. For the purpose of notice, the respective parties shall be served by certified or <br /> registered mail, return receipt requested, addressed to the COUNTY or the HOSPITAL at <br /> the addresses listed below: <br /> Orange County <br /> c/o Orange County Director of Social Services <br /> P.O. This Agreement contains the entire understanding of the parties and shall not <br /> be altered, amended, or modified, except by an agreement in writing executed by the duly <br /> authorized officials of both parties. <br /> 2 <br />
