Passmore, ext. 2000 <br /> TELEPHONE NUMBER <br /> Hillsborough 732-8181 <br /> Chapel Hill 968-4501 <br /> Mebane 227-2031 <br /> Durham 688-7331 <br /> ----------------------------------------------------------------------- <br /> PURPOSE: <br /> The purpose of this Agreement with the University of North Carolina <br /> (UNC) Hospitals for $45, 500 is to continue the Wellness Program <br /> initiated by foundation funding secured by the Friends of the Chapel <br /> Hill Senior Center, Inc. for the past three years <br /> BACKGROUND: <br /> In 1994 the Friends of the Chapel Hill Senior Center received funding <br /> to implement a Wellness Program at each of the Orange County Senior <br /> Centers . This three year project, funded by the Kate B. Reynolds <br /> Foundation, was due to end June 30, 1997 unless alternate funding <br /> arrangements could be made. Due to the success of the Program, UNC <br /> Hospitals proposes to fund the Wellness Coordinator position through <br /> contractual agreement with Orange County. This will expand existing <br /> programs and develop and implement new programs to serve the health and <br /> wellness needs of seniors in this County. The Friends as well as the <br /> United Way will also continue to provide administrative support for <br /> this public/private partnership. <br /> RECOMMENDATION: <br /> The Manager recommends approval and the Chair to sign Agreement between <br /> UNC Hospitals and Orange County for the continuation of the Wellness <br /> Program. <br />
