Contract#68-1020 <br /> Assistance Travel&Transport,Inc. <br /> ATTACHMENT N <br /> OUTCOMES AND REPORTING <br /> Orange County Department of Social Services <br /> By signing and submitting this document,the Contractor certifies that it agrees to the following: <br /> 1. The Contractor agrees to participate in program, fiscal and administrative monitoring and/or audits, <br /> making records and staff time available to Federal, State and County staff. <br /> 2. The Contractor agrees to take necessary steps for corrective action, as negotiated within a corrective <br /> action plan,for any items found to be out of compliance with Federal,State,and County laws,regulations, <br /> standards and/or terms of the Contract. <br /> 3. The Contractor agrees that continuation of and/or renewal of this Contract is contingent on meeting the <br /> following requirements. The Contractor agrees to: <br /> A. Adhere to all Federal and State transportation laws&regulations. <br /> B. Transport clients to and from: (1)Work,(2)Daycare, (3)Job Interview,(4)Job Fair,(5) <br /> Vocational Training,and/or other destinations as requested by the County. <br /> C. Transport approved, authorized clients to specified destinations within and outside of Orange <br /> County upon request,on time,and in accordance with arrangements made by the County. <br /> D. Transport clients only to approved destinations.Any additional destinations cannot be provided <br /> without the expressed consent and authorization of the County. <br /> E. Provide safe,reliable and on-time transportation to clients. <br /> F. Maintain sufficient records to document that service was provided. Such documentation shall <br /> include:(1)name of the driver who provided each trip,(2)the vehicle number used to provide <br /> the trip,(3)the client's name(4)the beginning location and ending destination for each trip. <br /> G. Allow await time often(10)minutes before classifying a client as a no-show. <br /> H. Notify the County of any no-shows as soon as possible. <br /> I. Provide a separate monthly invoice for each client. <br /> J. Not enter into a client's home for any reason. <br /> 4. To facilitate the work of the Contractor,the County agrees to: <br /> A. Have the client contact the Contractor and provide(1)name, (2)address,(3)phone number, (4) <br /> appointment time,(5)pick up location,and(6) destination. <br /> B. Notify the Contractor of any plans to discontinue service within five(5)calendar days of the <br /> proposed action. <br /> C. Notify the Contractor at least 4 business hours prior to scheduled appointments. <br /> D.
