STATE OF NORTH CAROLINA 2 <br /> COUNTY OF ORANGE <br /> RENEWAL AGREEMENT BETWEEN <br /> ORANGE COUNTY HEALTH DEPARTMENT <br /> AND <br /> JOINT ORANGE-CHATHAM COMMUNITY ACTION AGENCY <br /> THIS AGREEMENT,made and entered into this the 1 st day of January, 1997 by and between the Orange <br /> County Health Department,hereinafter referred to as the"OCHD,"and the Joint Orange-Chatham Community <br /> Action Agency,hereinafter referred to as the"JOCCA." <br /> WITNESSETH: <br /> WHEREAS,an agreement was made and entered into as of the 1st day of October, 1995 by and between the <br /> OCHD and JOCCA to implement the goals of the W.K. Kellogg Foundation Community-Based Public Health <br /> Project in Orange County. <br /> NOW, THEREFORE, in consideration of the premises and of the following mutual promises, covenants,and <br /> conditions, OCHD and JOCCA agree as follows: <br /> 1. The Agreement will be continued ot'i a month to month basis beginning January 1, 1997 for a period not <br /> to exceed three months. <br /> 2. Payment shall not exceed$6,878.09. <br /> 3. All equipment acquired with funds allocated by this agreement and preceding agreements shall be <br /> returned to OCHD upon final payment. <br /> 3.
