Page 6 Orange County <br /> Contract Addendum <br /> Contractors will be notified of performance-based adjustments in January 1998. Decreases to the <br /> budget will not exceed ten percent. The resulting adjusted budget will be the baseline for the <br /> following fiscal year. Performance will be based on the latest screening data available on <br /> December 31,1997, for the prior fiscal year (FY 1996-97). If additional funds are available, <br /> eligible contractors will be notified in February 1998. <br /> Performance-based adjustments to restore decreased fiscal year 1997- 98 budgets will be made in <br /> September 1997. Restoration of funds will be based on updated performance indicators using data <br /> reported through September 1997, for fiscal year 1996-97. <br /> B. Voluntary Adjustments: <br /> Midyear, voluntary increases or decreases may be made in the baseline budget of contractors. <br /> Those whose pattern of expenditures predict a surplus of funds in the contract year may be asked <br /> to approve a one-time, voluntary budget rev..on (decrease) to free up excess funds which may be <br /> allocated to another contractor. By the same token, contractors whose pattern of expenditures <br /> predict a shortage of funds in the contract year may be asked to approve a one-time, voluntary <br /> budget revision (increase) to cover anticipated needs. In either event, the allocation for the <br /> baseline budget for the following fiscal year will be restored to the adjusted levels determined in <br /> accordance with Section VII. A., above. <br /> VIII. SUMMARY OF D_ FL.IVERABLES <br /> A. Breast and Cervical Cancer Screenings will be provided to at least a total of= women during <br /> the contract period. Performance indicators will apply to the total women screened by the <br /> contractor, regardless of the established target levels. The contractor will assure age- and risk- <br /> appropriate breast and cervical cancer screenings and follow-up in the following manner: <br /> At least 75% X122) of the total women screened during the contract period will be 50 years of age <br /> or older. <br /> No more than 25%(43) of the total women screened during the contract period may be age 49 <br /> years or younger, of w ich 7% (12). of the total women screened may be under age 40 years. <br /> Appropriate to the age and risk status of the women, and consistent with the cervical and <br /> breast health protocols issued by the DEHNR, 100% of the women screened will receive both <br /> cervical cancer and breast cancer screening services. <br /> B. 100% of women having abnormal breast or cervical screening results will be followed up and <br /> documentation of final diagnostic disposition will be made and reported to the BCCCP within 60 <br /> days of the initial screening date. <br /> G--Documentation of the treatment disposition for 100% of those women diagnosed with breast or <br /> pre-invasive/invasive cervical cancer will be made and reported to the BCCCP within 30 days of <br /> the final diagnostic disposition. <br /> D. Data entry and/or submission of data will be made no later than the 15th of each month for the <br /> previous month's services. <br /> E. A system for contacting women for annual rescreening will be in place with an emphasis on <br /> rescreening women over 50 years of age. <br /> ealth Departme t Signature-Health Director Date <br />
