All enrolled children, regardless of Child Service Coordination Provider Agency, will have Identification <br /> and Referral Forms (DEHNR 3748) and Status Report Forms (DEHNR 3750) entered into the Health <br /> Services Information System (HSIS) at the local health department in a timely manner upon receipt from <br /> outside agencies. <br /> 18. Signed Provider Agreements for Child Service Coordination Agencies will be maintained for all county <br /> providers participating in the program. Agreements will be updated annually. <br /> 19. Child Service Coordination staff must meet the minimum staff qualifications as specified in the Child <br /> Service Coordination Program Technical Manual. <br /> 20. Child Service Coordinators working full-time in the Child Service Coordination Program will have no <br /> more than 75 families in their caseload at a given time. Employees working part-time in Child Service <br /> Coordination will have a number less than 75 prorated for the time spent in Child Service Coordination <br /> activities. <br /> 21.
