CSCP and WIC staff must jointly <br /> decide on estimated % to be used on both WIC and CSCP contract addenda. (The percentage for this <br /> item must be the same as for the corresponding item on the WIC Contract Addendum.) <br /> Projections for CSCP clinical objectives #10 through #16 should range from 90 to 100 percent. <br /> If the performance level is below 90 percent, a written explanation by the health department <br /> must be attached. <br /> 10. 95 % of all enrolled children will have an assessment of parent-child interaction at mandated face-to- <br /> face contacts. <br /> 11. 95 % of all enrolled children will have a review of immunizations and referral and follow-up as <br /> necessary. <br /> 12. 93 % of all enrolled children will receive at least mandated minimum contacts by the designated Child <br /> Service Coordinator including: <br /> a. Review and update of risk indicators <br /> b. Assessment of current needs <br /> c. Update of care coordination plan <br /> d.
