(CHTOP) in the <br /> amount of$12;878.00 to the Orange County Health Department to provide supportive services to <br /> the Family Resource Center in Northern Orange County. <br /> Background: <br /> The CHTOP has received an emergency grant from the Local Smart Start Board to administrator <br /> three family resource centers in Orange County. The emergency was created when a former <br /> contractor to the Smart Start Board failed to maintain its agreement. As a partner in the Local <br /> Smart Start Board,the Health Department was asked to assist the Family Resource Center in <br /> collaboration with CHTOP. This request was justified in part because the Health Department, <br /> through the Community-Based Public Health Initiative, had assisted in getting the Family <br /> Resource Center in Northern Orange County started. The attached agreement has been evolving <br /> since we began our collaborative work in January, 1997. The agreement provides the Health <br /> Department funding for a half-time health educator to work with the center coordinator and the <br /> Family Resources Center Board to accomplish the following outcomes: <br /> 1. Building leadership capacity of community members through the Community Voices <br /> Program. <br /> 2. Coordination of programming provided by the Health Department and other resources, <br /> e.g. Health Screening, Well Child exams, immunizations, and classes on health and <br /> other topics of interest to the community. <br />
