STATE OF NORTH CAROLINA — RETURN THIS COPY TO THE CLERK'S OFFICE COUNTY OF ORANGE FOR THE PERMANENT AGENDA FILE RENEWAL AGREEMENT BETWEEN oi-c- ORANGE COUNTY HEALTH DEPARTMENT AND THE UNIVERSITY OF NORTH CAROLINA AT CHAPEL HILL For Its Department of Family Medicine In The School of Medicine THIS AGREEMENT,made and entered into this the 1 st day of January, 1997 by and between The University of North Carolina at Chapel Hill,hereinafter referred to as "The University," for its Department of Family Medicine in the School of Medicine and the Orange County Health Department,hereinafter referred to as the "OCHD." WITNESSETH: WHEREAS, an agreement was made and entered into as of the 1 st day of January, 1997 by and between the Orange County Health Department and The University of North Carolina at Chapel Hill for its Department of Family Medicine in the School of Medicine. NOW,THEREFORE, in consideration of the premises and of the following mutual promises, covenants, and conditions, OCHD and The University agree as follows: 1. The Agreement will be continued on a month to month basis beginning January 1, 1996 for a period not to exceed six months. 2. All other terms and conditions shall remain the same.
