To approve the Home and Community Care Block Grant Funding Plan. <br /> BACKGROUND: The State Department of Human Resources established the Home and <br /> Community Care Block Grant which became effective July 1, 1992. The grant did not create new <br /> funding streams, but combined funds appropriated to the Department on Aging, Department of Social <br /> Services, and Joint Grange Chatham Community Action Agency to administer programs for senior <br /> citizens. <br /> The block grant requires the Board of Commissioners to approve the recommended funding plan. The <br /> recommended funding plan for the 1998-99 fiscal year is $344,825. This amount is $6,453 more than <br /> 1997-98 grant amount of$335,372. The Home and Community Care Advisory Board reviewed the <br /> plan on May 18, 1998. The Funding Plan is attached. <br /> RECOMMENDATION(S): The Manager recommends that the Board approve the 1998-99 Home <br /> and Community Care Block Grant Funding Plan and authorize Chair to sign grant documents. <br />
