coordinate on-site data collection. <br /> 2. Site staff will be responsible for most data collection. <br /> Other Items <br /> 1. Each site will identify a project leader/liaison for HealthMETRICS. <br /> 2. Site-specific data will be held strictly confidential. <br /> 3. HealthMETRICS Will own the data from this study. <br /> 4. HealthMETRICS Will use data as a benchmark for other providers but will not reveal sites or <br /> corporate affiliation of sites. <br /> 5. HealthMETRICS owns the rights to all survey instruments and measurement tools used to <br /> support this study. <br /> 6. HealthMETRICS guarantees participant satisfaction with services and that this effort <br /> will generate savings in excess of the project costs. <br /> 7. Timetable for project completion will be set in consultation with management of each <br /> county. (See Project Timeline below.) <br /> 8. The recommendations that will be made by HealthMETRICS in the report are specific to the <br /> site where information will be collected. Any decisions to implement or not to implement <br /> any of these recommendations will be solely the responsibility of the site and Its staff. <br /> Further, HealthMETRics disclaims all responsibility for any expenses, losses, or action <br /> incurred or undertaken by the health center as a result of the sites' receipt or use of the <br /> information contained in the report. <br /> 9. The information contained in the report will be provided to management by HealthMETRICS <br /> subject to the following conditions: (1) That management use such information solely for <br /> its own internal business purposes;(2) That management not disclose, sell, loan, transfer <br /> or otherwise distribute such Information to third parties; (3) That management use its best <br /> efforts to prevent the disclosure, sale, loaning, transfer or other distribution of such <br /> Information by the organization's employees, agents, and independent contractors; and (4) <br /> That management not reproduce or copy such information without HealthMETRICS' prior <br /> written permission In each case, except as reasonably required to accomplish such <br /> management's own internal business purposes. <br /> Project Timeline <br /> After conclusion of the contract and identification of the participating sites, the project timeline is <br /> as follows: <br /> 1. Initial phone meeting with site project liaison and participating site project leaders: time <br /> depends on the availability of staff. <br /> 2. Site visit to individual sites: time will depend on the availability of site staff. A site visit <br /> usually takes 4-6 hours. <br />
