Exhibit A <br /> Ref: Project Scope and Costs <br /> HealthMETRics Deliverables <br /> 1. Provide participating sites with service specific tools for measurement of: <br /> A. Patient satisfaction <br /> B. Clinical indicators <br /> C. Staff satisfaction <br /> D. Unit cost <br /> 2. Train designated on-site staff on data collection techniques. HealthMETRres will assist clinic <br /> staff with data collections when necessary. <br /> 3. Produce an individual, comprehensive report to each participating site, including; <br /> A. A process description and site data on: <br /> i. Patient satisfaction <br /> H. Clinical indicators <br /> iii. Staff satisfaction <br /> iv. Unit cost <br /> B. Comparison to other providers within the HealthMETRres adult health care database <br /> and the Best Practice identified. Data is adjusted for regional economic differences <br /> and demographics. <br /> C. A detailed description of the Best Practice's operations including, but not limited to: <br /> i. Staffing structure <br /> ii. Job responsibilities <br /> iii. Patient flow <br /> iv. Clinic layout <br /> v. Scheduling structure <br /> vi. Other key systems <br /> D. Site-specific, actionable recommendations to change the site process when changes <br /> would generate measurable improvements in outcomes. Recommendations are <br /> based on the detailed comparison of a site's process and outcomes to those of the <br /> Best Practice. <br /> HealthMETRres will provide five copies of the report per participating site. <br /> 4. A presentation to review and discuss the findings will be made at the conclusion of the <br /> project. <br /> 5. A follow up call will be made 12 months post final report presentation to ascertain the <br /> changes made due to project recommendations and estimated cost savings. A letter <br /> documenting this information to be sent to the site after the follow up call. <br /> Participating Site SuRRort <br /> 1.
