Exhibit A <br /> REF: Consultant Project: Increase the Nutrition Quality of Foods Offered at the Triangle <br /> SportsPlex <br /> Goals: <br /> 1) To improve the nutrition quality of foods offered at Triangle SportsPlex while maintaining <br /> revenue and the current level of customer service. <br /> 2) Provide healthy options at SportsPlex snack bar using Nutrition Standards established by the <br /> Alliance for a Healthier Generation (AHG). <br /> 3) Provide desirable food items to maintain snack bar revenue. <br /> 4) Implement an education program to staff and visitors to include newsletter articles and <br /> poster campaign in the facility. <br /> Provider (RD) Will: <br /> 1) Analyze nutrition info for all products currently being served and provide written analysis to <br /> Orange County Health Dept (OCHD) and SportsPlex. <br /> 2) Will provide survey for SportsPlex to distribute to members regarding proposed snack bar <br /> changes. <br /> 3) Provide written recommendations for which products to keep, which to replace and which to <br /> eliminate based on nutrition and sales information. <br /> 4) Recommend pricing structure to incentivize the purchase of healthy options. <br /> 5) Provide 12 newsletter articles to include in the SportsPlex Newsletter during the next year. <br /> 6) Provide tool kit to educate SportsPlex members and staff on basic nutrition principles. <br /> OCHD will: <br /> 1) Serve as liaison between RD and management of SportsPlex. <br /> 2) Coordinate with management of SportsPlex as needed. <br /> 3) Provide assistance as needed to include the help of interns to gather and compile data. <br /> Fee: <br /> RD consultant will receive $2500 for project. RD will provide billing to OCHD and is to receive <br /> two payments of $1250. Project is to start on May 14, 2013 and will be completed by June 30, <br /> 2013. This completion date is dependent upon RD receiving information in a timely fashion. RD <br /> will not be held responsible for project running past due date if RD cannot receive information <br /> required to complete project. This fee includes: <br /> 1. All deliverables listed in this document. <br /> 2. Four RD visits to facility or Orange County Health Department. <br /> 3. Four educational posters of RD choice. <br /> 4. Twelve newsletter articles to be included in SportsPlex Newsletter. <br /> 5. Teaching tool kit for staff of SportsPlex. <br /> The cost of any additional materials or trips to the Orange County Health Department or <br /> SportsPlex will be at cost of OCHD- <br /> Maria Kennedy,RD,LLC May 1,2013 <br />
