EXHIBIT A <br /> SOFTWARE LICENSE AGREEMENT-INSTALLATION SERVICES <br /> A. Accela shall provide the following installation services relating to the"Software",up to 788 hours for <br /> 1. Combination Building Permit-60 hours <br /> 2. Building Permit-24 hours <br /> 3. Plumbing Permit—12 hours <br /> 4. Electrical Permit—12 hours <br /> 5. Mechanical Permit—12 hours <br /> 6. Certificate of Occupancy—4 hours <br /> 7. Code Enforcement Violation/Complaint—24 hours <br /> 8. Subdivision Permit—16 hours <br /> 9. Land Use Permit—12 hours <br /> 10. Conditional Use Permit—8 hours <br /> 11. Fire Prevention Inspection—24 hours <br /> 12. Septic Site Evaluation—12 hours <br /> 13. Well Permit-12 hours <br /> 14. Erosion Control—12 hours <br /> e.Telephone/Management Support 29 hours <br /> f. Implementation Consultant 117 hours <br /> g.Workflow assistance in setup 30 hours <br /> i. Valid Street Name/Creation/Conversion 20 hours <br /> j. Contractor Database Assistance 20 hours <br /> (If more than one department or functional group,add 25 hours each) <br /> B. Accela shall be paid for Installation Services at the prevailing rate plus reasonable expenses,in addition to the License Fee for the"Software". <br /> 1. Installation Services are estimated at 788 hours at$92.00 an hour for a total of$72,496.00. <br /> 2. Assistance Loading Client Server Database for up to 50 hours @$140.00/hr.for a total of$7,000.00. <br /> 3. Assistance with`PERMITS'T)rueGis'"for up to 50 hours @$140.00/hr.for a total of$7,000.00. <br /> 4. Assistance with`PERMITS'Offieelink—for up to 3 hours @$92.00/hr.for a total of$276.00. <br /> 5. Assistance with`PERMITS'Voice'"for up to 15 hours @$92.00/hr.for a total of$1380.00. <br /> C. Assessor's Database Conversion(One fixed length record input format)$1.400.00. <br /> Note:Accela has estimated assistance hours based on the County converting the assessor information. Accela shall supply all formatting information. <br /> Please note that if the County incorrectly alters the data,the County will be responsible for time and materials for Accela to correct the data. <br /> Please keep in mind that this approach is not recommended. If the database structure changes within'PERMTS'PIUS®,the County will be <br /> responsible for preparing a new assessors conversion or contract with Accela to provide one. The current cost for Accela to provide an assessors <br /> parcel conversion is$4,900.00. <br /> D. Permits History Conversion(205 hours @$92.00/hr)$18.860.00. <br /> Note: This is based on the history conversion information that the County supplied to Accela. <br /> E. Accela shall submit monthly invoices to User detailing the installation services provided and the time and expense incurred. User shall pay such <br /> invoices within thirty(30)days. <br /> F. Accela shall be paid for User and Technical Training at User site. <br /> 1. User Training classes shall be for four(4)days at a total of$_4,40Q.00 plus travel and expenses at User site. <br /> Accela shall submit an invoice when the training is completed. User shall pay such invoice within thirty(30)days. <br /> G. The total estimated reasonable and customary expenses are$18.400.00 and shall not be exceeded except upon prior written consent of User. <br /> Travel Time will be billed portal to portal,at our then current prevailing rate. <br /> H. Accela shall submit monthly invoices to User detailing the modifications provided and the time and expenses incurred. User shall pay such invoices <br /> within thirty(30)days. <br /> 1. Modifications are estimated at$ N A .The total amount for the modifications shall not exceed$ N A except upon prior written consent of <br /> User. Accela will retain the exclusive rights to all modifications. These modifications may be utilized at Accela's discretion. <br /> 1.
