/ <br /> Community Based Public Health Initiative <br /> Grant Project Ordinance <br /> Program Number 11-644 <br /> Be it ordained by the Orange County Board of County Commissioners that pursuant to Section 13.2 of <br /> Chapter 159 of the General Statutes of North Carolina,the following grant project is hereby adopted. <br /> Section 1. The project authorized is the Community Health Advocacy Program(CHAP)as awarded to the <br /> Orange County Health Department by the Kellogg Foundation. The purpose of this four year <br /> grant is to improve the health of minority populations in targeted communities. <br /> Section 2. The officers of the County are hereby directed to proceed with the grant project within the grant <br /> document and the rules and regulations of the Kellogg Foundation. <br /> Section 3. The following revenue is anticipated to be available to complete this project: <br /> 1992-93 Grant Funds- CHAP $52,820 <br /> 1993-94 Grant Funds - CHAP $93,884 <br /> 1994-95 Grant Funds-CHAP $93,626 <br /> 1995-96 Grant Funds-CHAP $82,160 <br /> 1996-97 Grant Funds-CHAP $33,750 <br /> 1997-98 Grant Funds-CHAP $11,250 <br /> 1998-99 Grant Funds-Community Health Initiative $30,028 <br /> 1999-00 Grant Funds-Community Health Initiative $20,463 <br /> 2000-01 Grant Funds-Community Health Initiative $36,717 <br /> Total CHAP Grant Funs $454,698 <br /> Section 4. The following amount is appropriated for this project: <br /> Human Services- Community Based Public Health Initiative $454,698 <br /> Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient specific <br /> detailed accounting records to provide the accounting to the grantor agency required by the <br /> grant agreement and federal and state regulations. <br /> Section 6. Funds may be advanced from the General Fund for the purpose of making payments due. <br /> Reimbursement_requests should be made to the grantor agency in an orderly and timely <br /> manner. <br /> Section 7. Copies of this grant project ordinance shall be made available to the finance officer for <br /> direction in carrying out this project. <br />
