MEMORANDUM OF AGREEMENT BETWEEN <br /> THE UNIVERSITY OF NORTH CAROLINA AT CHAPEL HILL <br /> CHAPEL HILL,NORTH CAROLINA <br /> for the <br /> DEPARTMENT OF DENTAL ECOLOGY <br /> in the <br /> SCHOOL OF DENTISTRY <br /> and <br /> Orange County Health Department <br /> I. PURPOSE <br /> This document is a Memorandum of Agreement between The University of North Carolina at Chapel Hill, <br /> for its Department of Dental Ecology in the School of Dentistry, hereafter referred to as the "University," <br /> and Orange County Health Department, hereafter referred to as the "OCHD",for the purpose of providing <br /> a Children's Dental Care Coordinator. <br /> 11. EFFECTIVE DATES <br /> This agreement shall be effective as of April 1, 2000 and shall remain in effect until June 30, 2000 unless <br /> terminated as set forth below. Either party may request a change in the terms of this agreement. Any <br /> change to this agreement must be approved by both parties'written endorsement thereto. Either party may <br /> terminate this agreement by giving the other party 30 days written notice in advance of the desired <br /> termination date. <br /> III. WITNESSETH <br /> WHEREAS, The Health Department desires the services of one adjunct faculty to serve as Children's <br /> Dental Care Coordinator in the Dental Service of the Orange County Health Department for the academic year <br /> 1999-2000; <br /> WHEREAS, The Health Department desires to reimburse the School of Dentistry for the adjunct faculty's <br /> time commitment; <br /> WHEREAS, The School of Dentistry wishes to provide the services of one adjunct faculty to the Health <br /> Department; <br /> NOW, THEREFORE, in consideration of the premises and of the following mutual promises, covenants, <br /> and conditions, the School of Dentistry and the Health Department agree as follows: <br /> 1. The Health Department will reimburse The School of Dentistry by June 30,2000, $15,000.00. <br /> 2. The School of Dentistry will provide the services of one adjunct faculty to the Health Department <br /> for the purpose of serving as the Children's Dental Care Coordinator,and the Coordinator will: <br /> a. Organize the screening assessment program in all the daycare centers and family child care <br /> homes in Orange County as time permits. <br /> b. Coordinate a comprehensive dental education program for parents and caregivers(i,e, <br /> pediatricians,daycare staff,health department staff)of children ages 0-5. <br /> c. Implement the"Tooth Time"'dental education module for preschool children which has <br /> already been developed and is being utilized in other counties(Catawba, Guilford). <br /> d. Coordinate a plan of treatment for income-eligible children. <br /> 3. School of Dentistry will bill The Health Department $15,000.00 by June 15, 2000 for the <br /> salary/fringe of one adjunct faculty. <br /> 4. The person whose services are to be provided pursuant to the Agreement is, for all purposes an <br /> employee of the University of North Carolina School of Dentistry. <br />
