U.S. EQUAL EMPLOYMENT OPPORTUNITY COMMISSION <br /> Washington, D.C. 20507 <br /> DESIGNATION OF CONTRACT MONITOR <br /> Contract No. 0/5010/0145 Contractor <br /> Orange County Human Relations Commission <br /> 110 South Churchton Street <br /> Hillsborough, NC 27278 <br /> Supplies/Services FEPA CONTRACT <br /> In connection with the discharge of my responsibilities as Contracting Officer <br /> under the above numbered contract, I hereby designate Reuben Daniels, Acting , <br /> Director, Charlotte District Office as Contract Monitor to act on my behalf in <br /> carrying out the specific duties listed below: <br /> 1 Monitor the Contractor's day-to-day performance to assure compliance with <br /> the technical requirements of the contract. This includes providing the <br /> Contractor with technical direction. Such direction is to remain within the <br /> scope of the contract as it is written. <br /> 2. Review and approve progress reports, technical reports, and all other <br /> materials provided by the Contractor as called for by the contract. Notify <br /> the Contracting Officer and Supervisor if reports or other items submitted <br /> are to be rejected and state basis for rejection. <br /> 3. Complete attached progress report Quarterly and FINAL and forward to the <br /> Contracting Officer. <br /> 4. Certify the Contractor's invoices for payment, including any credit due the <br /> Government. This includes the authority to request the Contractor to make <br /> corrections or submit new invoices when clerical errors are detected. If a <br /> contractor fails to submit a requisite invoice based on actual production and <br /> subsequently submit an invoice for another advance payment, the contract <br /> monitor must certify that the contractor is producing on schedule before <br /> that advance payment is paid. <br /> 5. Keep detailed records of the Contractor's performance, and if it is not <br /> satisfactory or if problems are anticipated, notify the Contracting Officer and <br /> Supervisor, in writing, as to the cause and recommend a course of action <br /> from a technical standpoint. Immediate notification is essential to assure <br /> that the Contracting Officer takes appropriate action to protect the <br /> Government's rights under the contract. <br />
