Exhibit B-Scope of Services-Fiscal Year 2012-13 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $12,000 <br /> Agency: Charles House Association <br /> Program <br /> Charles House Daytime Eldercare Center <br /> Purpose <br /> Charles House serves older adults with cognitive and/or physical disabilities who are <br /> continuing to live within the community with the support of families and caregivers. Charles <br /> House has been responding to the need for community eldercare support for twenty one years, <br /> having opened in 1990. Over the years,the population striving to maintain loved ones at home and <br /> in the community has continued to grow; it is anticipated that as the population continues to age <br /> and grow in numbers,the need for Charles House to support families and provide enriching social <br /> programs for seniors will increase significantly. <br /> Charles House conducts a program of activities, services and care Monday through Friday, <br /> from 7:30 to 5:30, and Saturdays from 9:00 to 4:30 for up to 22 participant clients per day, <br /> projecting to serve approximately 70 families in the 2012/13 Outside Agency Funding year, <br /> with fee assistance being provided to 15 to 20% of the client families. Charles House services <br /> support families as caregivers and enriches the lives of their family members. <br /> Outcomes <br /> The overall results the Charles House program strives to achieve include: <br /> Improved quality of life for participants and caregiving families, <br /> Families being supported in their choice to care for aging loved ones, (family members <br /> being able to continue employment while being caregivers, others finding much <br /> needed respite, families not feeling alone in their decision to be caregivers), <br /> Charles House participants will function at their highest capacity—socially, physically, <br /> and cognitively—supported through their participation in Charles House program. <br /> The measurable outcome objectives that attempt to target these program goals/outcomes <br /> include: <br /> • 50% of enrolled families in the funding year will have the capacity to continue providing <br /> care in the home and not require, or delay, residential care placement for their <br /> participant/enrollee. <br /> • Over 50% of families will report in the annual evaluation survey satisfaction with the <br /> Charles House program and will report positive effects of the program for their enrolled <br /> family members, such as improvement in their participant/enrollee's behavior, emotional <br /> status, physical and/or cognitive functioning. <br /> • Charles House Association will continue to surpass the NC Standards and maintain state <br /> certification to operate as an adult day care program. <br /> Program Budget <br /> The intended use of these grant funds is to off-set agency payroll expenses in order to clients <br /> enrolled and receiving financial assistance from the Scholarship Fund. <br /> Certified b v`�- Title: exeiu. e Aeeecgr Date: 62- ae/? <br />
