_ RETURN THIS COPY TO THE CLERK'S _ <br /> OFFICE FOR THE PERMANENT AGENDA FILE <br /> 4) <br /> STATE OF NORTH CAROLINA <br /> COUNTY OF ORANGE <br /> AGREEMENT BETWEEN <br /> THE UNIVERSITY OF NORTH CAROLINA AT CHAPEL HILL <br /> SCHOOL OF DENTISTRY <br /> FOR ITS <br /> DEPARTMENT OF DENTAL ECOLOGY <br /> AND <br /> ORANGE COUNTY HEALTH DEPARTMENT DENTAL SERVICE <br /> AS PART OF THE COUNTY OF ORANGE <br /> THIS AGREEMENT is made and entered into this 1s day of July, 2001 by and between The University <br /> of North Carolina at Chapel Hill, School of Dentistry, for its Department of Dental Ecology; and The <br /> Orange County Health Department hereinafter referred to as"The Health Department." <br /> WITNESSETH: <br /> WHEREAS, the Health Department desires the services of one faculty to direct the Dental Service of the <br /> Orange County Health Department for the fiscal year 2001-2002, and <br /> WHEREAS, the Health Department desires to reimburse The School of Dentistry for the faculty's time <br /> commitment; <br /> NOW, THEREFORE, in consideration of the premises and of the following Mutual promises, covenants <br /> and conditions,The School of Dentistry and The Health Department agree as follows: <br /> 1. The Health Department will reimburse The School of Dentistry at the close of each quarter <br /> for 1/4 of$10,000. <br /> 2. The School of Dentistry will provide the services of one faculty to The Health Department for <br /> the purpose of directing the dental program in the Health Department. <br /> 3. School of Dentistry will bill The Health Department $2,500 at the close of each quarter for <br /> the salary. <br />
