Proposal for Construction Materials Testing.Services Irerracon <br /> Walnut Grove Convenience Center Expansion m Cedar Grove, North Carolina <br /> August 15, 2012 c Terracon Proposal No. P70120381 <br /> Foundation related services will include: <br /> • Observing footing excavations relative to size,depth, and presence of unsuitable soils; <br /> • Evaluating bearing soils at the base of selected foundations by performing dynamic cone <br /> penetrometer field testing procedures and <br /> Observing placement of reinforcing steel relative to number, size and spacing of bars <br /> Concrete related services: <br /> Field sampling and testing of concrete for slump, air content and temperature; <br /> u Sets of 4 (6x12 inch) cylinders will be cast every 50 cubic yards poured or daily for each <br /> concrete mix; and <br /> Transporting, curing and laboratory testing concrete cylinders at frequency specified; <br /> Load Testing of Post-installed anchors <br /> The above services will be provided under an as-requested basis. Additionally, we request a <br /> 24 hour notice to schedule our services. If you would like us to perform any additional work, <br /> please contact us and we will issue a proposal amendment that outlines the additional work to <br /> be performed and associated fees, <br /> C. COMPENSATION <br /> Observation and material testing services provided will be billed in accordance with the line item <br /> unit rates illustrated on the attached Exhibit A — Cost Estimate. Based on the scope of <br /> services, we estimate a cost of approximately $13,146;00 for the quantity and rates illustrated <br /> herein. <br /> Please note that this is only a budget estimate and not a not-to-exceed price.An overtime <br /> premium of 1.5 times the hourly rate will apply for services provided.Monday through Friday that <br /> are in excess of 8 hours per day and for services provided before 6:00 AM and after 7:00 PM, <br /> as well as for services provided on Saturday, Sunday and Terracon recognized Holidays. The <br /> quantity of the work performed and factors beyond our control, such as weather and the <br /> contractor's schedule/efficiency, will affect the actual cost for our services. <br /> You will be invoiced at regular intervals but not less than monthly for services performed. <br /> Invoices for our services will be directed to the addressee of this letter unless we are directed <br /> otherwise. Payment is due within thirty (30) days following receipt of an invoice. <br /> D.
