14 <br /> VISION BENEFITS <br /> Routine eye exams with UnitedHealthcare Choice Plus (POS) are subject to a co-pay and are <br /> covered every other year. With a Health Savings Account/High Deductible plan, all vision <br /> exams are subject to the deductible and co-insurance. <br /> Effective January 2012, employees became eligible to enroll in a vision plan with Community <br /> Eye Care. Employees who enroll pay a premium on a pre-tax basis. The plan includes a <br /> routine eye exam for a $20 co-pay and an $150 allowance for eyeglasses or lenses. <br /> 375 employees are currently enrolled in the vision plan.
